Intraventricular trifascicular block. Observations on conduction disturbance in bundle-branch system.

چکیده
A case of intraventricular trifascicular block secondary to an inferior myocardial infarction is reported. The case denonstrates instances of 2: I atrioventricular conduction due to alternating block in the right bundle-branch and block in the right bundle and both anterior and posterior divisions of the left bundle (trifascicular block). Instances of I: i AV conduction with grade I block and periods of type I (Mobitz) block are ascribed to a delayed and a Wenckebach type of conduction respectively in the posterior division of the left bundle.

Studies of heart block with His bundle electrograms.
A total of 24 patients with heart block revealed by scalar electrocardiography was studied using the His bundle electrogram. Four groups of disturbed conduction are described which include various types of heart block, partial and complete, at different sites, single and multiple, in the specialized conducting system. These are (i) intra-atrial block with delayed or blocked internodal conductio...
متن کاملAlternating and co-existing block in the divisions of the left bundle branch.
The tracings of two patients with acute myocardial infarction showing complete right bundle branch block and alternating block in the divisions of the left branch are presented. Both developed a type II (Mobitz) block which was attributed to a simultaneous interruption of conduction in the three main intraventricular tracts (trifascicular block). Left divisional blocks produced specific changes...
متن کاملSignificance of complete right bundle-branch block with right axis deviation in absence of right ventricular hypertrophy.
Complete right bundle-branch block with right axis deviation was seen in nine patients who did not have right ventricular hypertrophy, pulmonary disease, or extensive lateral myocardial infarction. Four patients had chronic block and five had acute myocardial infarction. This pattern was attributed to a coexisting block in the right branch and in the inferior division of the left branch. It fre...
متن کاملCardiac pacing in left bundle branch/bifascicular block patients.
The primary concern in patients with bifascicular block is the increased risk of progression to complete heart block. Further, an additional first-degree A-V block in patients with bifascicular block or LBBB might increase the risk of block progression. Anesthesia, monitoring and surgical techniques can induce conduction defects and bradyarrhythmias in patients with pre-existing bundle branch b...
متن کاملCoexistent Wenckebach phenomenon in the distal branches of the specialized conduction system.
A patient in whom Wenckebach phenomenon was observed to coexist in the three distal branches of the specialized ventricular conduction system, with an intermittent trifascicular block producing a second degree A-V block, is described. Surface electrocardiogram demonstrated the presence of this phenomenon in the right bundle branch (RBB) and the anterior division of the left bundle branch (LBB)....
